My Vaccine Record
NYC has now granted access to parents, guardians and individuals to the Citywide Immunization Registry (CIR). A new public website, My Vaccine Record. will allow members of the public to view and print their own and/or their children’s CIR record. Access is being rolled out initially only for holders of the New York City identification card, IDNYC, which all New York City residents can apply for online at www.nyc.gov/IDNYC.
The CIR is very helpful in making sure your child’s immunizations are always up-to-date; as you know, children can be excludable from school if they are missing vaccinations. If you have questions about the CIR, please visit their website, or call 311.
Registration for the 2017-18 school year
Registration for new students is daily at 8:30am.
In order to register your child, you must have the following with you:
- the CHILD whom you are registering
- the child’s *BIRTH CERTIFICATE*
- the child’s IMMUNIZATION RECORD (must be up-to-date with vaccines to attend school)
- TWO PROOFS OF ADDRESS – must reside in our school zone (see below for acceptable proofs and zone info)
- the child’s last REPORT CARD from their prior school
You must have all of the above information in order to register – we cannot register a student who is missing any documents. Please bring the originals and a copy for us to keep. Be advised that photo ID is required at all times to enter our school building.
In order to save time, you can print out the registration packet and bring it to registration completed. The medical form should be brought to your child’s pediatrician and returned to school ASAP. (immunization record is still required for registration)

Summer Book Lists
Book lists include books on different reading levels. Feel free to look at lists from various grades to select books on the appropriate reading level for your child. Your child’s June reading level was indicated on their end of the year report card.
